Billing for Midwives

CPT 59400 is being retired in 2027. What do the maternity billing changes mean for midwives?

Billing for Midwives is here to help you navigate this transition.

The biggest shift in maternity billing in 30 years is here.

Is CPT 59400 really going away?+
Yes, really. On January 1, 2027, CPT 59400 is being retired, along with all other bundled maternity codes, including 59425, 59426, and 59430.
Are CPT 59409 and 59410 going away?+
Yes, those are also going away. They will be replaced with new delivery-only codes, and new codes for a new category: labor management.
When do the changes take effect?+
January 1, 2027, but the transition affects antepartum billing and financial agreements in 2026.
Which maternity CPT codes are affected?+
A total of 16 maternity codes will be deleted. New codes, within a new framework of maternity billing, will replace them.
What's replacing them?+
New codes for labor management and delivery, and E/M codes for antepartum and postpartum.
Does this affect home birth and birth center billing?+
If you bill insurance for maternity care, yes. This is about the codes themselves, not the place of service.
Do I need to redo my fee schedule?+
That depends — do you already have a fee schedule by CPT code, based on logic and balanced such that the parts of the whole make sense? If not, you need to create one right away. Setting and Updating Your Fees includes tools to help you do just that. Either way, you'll need to update your fee schedule again for 2027. A new version of Setting and Updating Your Fees for 2027 will be available when that time comes. Having a schedule by CPT code in 2026 will make that easier, and is required in order to bill insurance during the transition time that is the rest of 2026.
Is this an evil plot against midwives?+
Not at all! In fact, codes for Cesarean and procedures that community midwives never perform are also being deleted. This change is about ALL maternity care, regardless of provider type.
